Invoice from the Undertaker addressed to the claimant. If there is a cash balance to be paid to the claimant, a bank stamped copy of the claimant's bank statement (must reflect the claimant's name and bank account number) is also required. The bank statement must not be older than 3 months. If the person claiming the cash portion is not the main member, spouse or beneficiary, a receipt in the name of the claimant from the Undertaker is required as proof that the claimant was responsible for the funeral costs. Alternatively, a Letter of Authority from the Magistrate's Court will be required. If neither is produced, an Estate Late cheque will be issued for the cash balance.

A copy of the policy contract / membership certificate, If the policy book or contract is lost, the client must complete an application for a duplicate which is to be sent with the claim documents. The application for duplicate fee can only be charged if the policy is to continue after the claim has been finalised.
